WAIVER and IMAGE RELEASE for The First Coast Beals Soundwalk
I agree, as a consideration of, and in consideration for, being permitted to participate in this Jonesport Soundwalk, to freely and expressly assume and accept any and all risks of injury or death to the participant, or property loss or damage, including injury, death, loss, or damage attributable to the negligence of The First Coast and the entire project team, the Town of Beals, the Beals Heritage Museum, and all affiliates.
I agree to release the sponsors and promoters of the Beals Soundwalk, the sponsors and promoters of The First Coast, and other sponsors or affiliated organizations, and their respective agents, directors, officers and employees from any and all responsibility or liability for injuries or damages to my person or property which result, either directly or otherwise, from my participation in the Beals Soundwalk and Exhibit event and the Beals Soundwalk walking tour, including injury, death, loss, or damage to my person or property, including injury, death, loss, or damage attributable to the negligence of The First Coast and the entire project team, the Town of Beals, the Beals Heritage Museum, and all affiliates and its agents, directors, officers and employees.
I agree not to make a claim against or sue The First Coast and the entire project team, the Town of Beals, Beals Heritage Museum, and all affiliates, or other sponsors of affiliated organizations and their respective agents, directors, officers and employees for injuries, loss, or damages related to my participation in the Beals Soundwalk and Exhibit event and the Beals Soundwalk walking tour.
I am aware that this is a release of liability and a contract between The First Coast and all of its affiliate organizations, and me. I am signing it freely and of my own accord and I recognize and agree that it is binding upon myself, my executor, heirs and assigns, and in the event that I am signing it on behalf of any minors, I have full legal authority to do so, and realize the binding effect of this contract on them, as well as on myself.
I hereby consent to The First Coast and affiliate organizations’ use of my image and name, and release The First Coast and the project team partners, and their officers, directors, agents and employees from any and all claims which may result at any time by reason of including, without limitation, claims of privacy.
I, THE UNDERSIGNED, HAVE CAREFULLY READ THIS RELEASE OF LIABILITY AND FULLY UNDERSTAND AND AGREE TO ITS CONTENTS.
